左卡尼汀在早产儿中的临床应用研究(1)

     【摘要】 目的 探討早产儿补充生理需要量的左卡尼汀对其脂肪酸代谢和体重增长的影响。 方法 60例早产儿, 随机分为治疗组及对照组, 各30例。治疗组在常规治疗的基础上于出生后开始给予左卡尼汀治疗, 对照组除了常规治疗外不给予任何干预。比较两组患儿出生后第1、5、10天血总胆固醇、甘油三酯水平;比较两组患儿恢复出生体重时间、出院体重、住院时间、游离肉碱水平;观察治疗组患儿用药不良反应。结果 两组患儿出生后第1、5天血总胆固醇、甘油三酯水平比较, 差异均无统计学意义(P>0.05);出生后第10天, 治疗组血总胆固醇及甘油三酯水平明显低于对照组, 差异均具有统计学意义(P<0.05)。两组患儿恢复出生体重时间、出院体重、住院时间比较, 差异均无统计学意义(P>0.05);治疗组游离肉碱水平为(34.7±8.0)μmol/L, 高于对照组的(11.0±4.7)μmol/L, 差异具有统计学意义(P<0.05)。治疗组患儿在应用左卡尼汀期间, 未发现有恶心呕吐、抽搐、肾功能异常、皮疹等不良反应, 所有患儿均治愈出院。结论 早产儿补充生理需要量的左卡尼汀有利于改善脂肪酸代谢, 改善肠外营养脂肪乳不耐受表现, 有利于游离肉碱水平维持在正常水平, 但不影响体重增长和住院时间。

    【Abstract】 Objective To investigate the effects of levocarnitine supplementation on fatty acid metabolism and weight gain in premature infants. Methods A total of 60 premature infants were randomly divided into treatment group and control group, with 30 cases in each group. The treatment group was treated with levocarnitine after birth on the basis of routine treatment, and the control group received no intervention except routine treatment. Comparison were made on total cholesterol and triglyceride levels on days 1, 5 and 10 after birth; recovery of birth weight, discharge weight, hospitalization time, free carnitine levels and adverse drug reactions between the two groups. Results Both groups had no statistically significant in total cholesterol and triglyceride levels on days 1, 5 and 10 after birth (P>0.05). On 10 d after birth, the treatment group had obviously lower total cholesterol and triglyceride level than the control group, and the difference was statistically significant (P<0.05). Both groups had no statistically significant difference in recovery of birth weight, discharge weight, hospitalization time (P>0.05). The treatment group had higher free carnitine level as (34.7±8.0) μmol/L than (11.0±4.7) μmol/L in the control group, and the difference was statistically significant (P<0.05). No adverse reactions such as nausea, vomiting, convulsions, renal dysfunction and rash were found in the treatment group during application of levocarnitine. All the children were cured and discharged from hospital. Conclusion The supplementation of L-carnitine with physiological requirement in premature infants can improve fatty acid metabolism, parenteral nutrition intolerance and conducive to the maintenance of normal carnitine levels free carnitine level, but it does not affect weight gain and hospitalization time.
