     [摘要] 目的 探讨血清C-反应蛋白(CRP)与白细胞计数(WBC)、红细胞沉降率(ESR)在评估肺部感染早期疗效的差异性。 方法 研究对象为临床资料完整的肺部感染患者63例,其中,中老年社区获得性肺炎(CAP)33例、慢性阻塞性肺疾病急性发作(AECOPD)合并肺部感染30例,另设健康查体者28例为对照组。检测患者治疗前及治疗后第3、7、14天和对照组CRP、WBC、ESR水平。 结果 CRP水平于肺部感染后迅速升高,当抗感染治疗有效时3 d后即大幅度下降;而WBC、ESR水平在肺部感染时有所升高,治疗有效后逐渐下降;治疗后第14天患者CRP水平降至正常,WBC水平大多数正常,而ESR水平大多数仍高于正常。 结论 CRP用于判断肺部感染早期疗效的价值大于WBC和ESR。

    Comparative study between C-reactive protein and white blood cell count, erythrocyte sedimentation rate in the early efficacy assessment of lung infection

    [Abstract] Objective To investigate the differences between the serum C-reactive protein (CRP) and white blood cell count (WBC), erythrocyte sedimentation rate (ESR) in assessing the early efficacy of lung infection. Methods The subjects were analyzed 63 patients with pulmonary infection, including 33 patients in the aged and elderly people with community-acquired pneumonia(CAP), 30 patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and pulmonary infection; 28 healthy people as control group. The CRP, WBC and ESR levels were detected before treatment and on the 3rd, 7th, 14th days after treatment. Results CRP levels rised rapidly in patients with pulmonary infection and declined significantly 3 days later after anti-infection therapy; While WBC, ESR levels in lung infection increased slowly and decreased gradually after effective treatment. On the fourteenth day after treatment, the patients′ CRP levels decreased to normal range, most patients′ WBC levels to normal range too, but many patients′ ESR levels were still higher than normal level.
