彩超用于微创保胆术治疗胆囊息肉术前诊断评估的 价值研究(1)
【摘要】 目的：分析彩色多普勒超聲检查在微创保胆术治疗胆囊息肉术前诊断评估中的应用价值。方法：选取100例微创保胆术胆囊息肉患者，术前均进行彩色多普勒超声检查，观察、记录胆囊形态、大小、胆囊壁厚度、息肉状况等，预测手术难易程度，针对手术困难组给予手术改进措施。结果：本组100例行微创保胆术治疗的胆囊息肉患者，术前彩超检查预测为手术困难的患者21例，21例均与手术中诊断一致，但漏诊2例，手术难度预测符合率为98.0%。手术困难组中转开腹1例(4.76%)，非手术困难组中转开腹0例，两组的中转开腹率对比，差异无统计学意义(P>0.05)。所有患者术后均无严重并发症，均痊愈出院。结论：在微创保胆术治疗胆囊息肉术前给予彩超检查，可对胆囊息肉样病变进一步确诊，准确评估手术难易程度，针对手术困难组给予手术改进措施，具有重要的临床指导价值。

【Abstract】 Objective：To analyze the value of color Doppler ultrasonography in the preoperative diagnosis and evaluation of minimally invasive cholecystectomy for gallbladder polyps.Method：100 patients with gallbladder polyp underwent minimally invasive cholecystectomy were examined by color Doppler ultrasonography before operation.The gallbladder shape，size，thickness of gallbladder wall and polyp status were observed and recorded to predict the degree of difficulty in operation.Surgical improvement measures were given to the difficult group.Result：100 patients treated with minimally invasive gallbladder-preserving surgery，the preoperative color ultrasound examination predicted that 21 cases were difficult to operate，21 cases were consistent with the diagnosis during surgery，but missed diagnosis of 2 cases，conformance rate of surgical difficulty prediction was 98.0%.And 1 case(4.76%) was transferred to open surgery in the difficult group，and 0 case was transferred to open surgery in the non-difficult group.Comparison of conversion laparotomy rates between the two groups，the difference was not statistically significant(P>0.05).All the patients were cured and discharged without serious complications.Conclusion：Color ultrasound examination is performed before minimally invasive cholecystectomy for gallbladder polyps can further diagnose polypoid lesions of gallbladder，accurately evaluate the degree of surgical difficulty，and provide surgical improvement measures for the difficult group，which has important clinical guiding value.
